Immunizations - Patient Refusal

Open the Patient History

From the HX list choose the Immunization History.

When screen opens, click on the Input History.

Choose the "Type of Vaccine" Magnifying glass to select from the vaccine types.

Choose from your list which vaccine the patient is refusing. This example is using Flublok for 18 Years and older.

Type in the Administered Date, or choose the clock icon to select the calendar.

Choose the Calendar from the pick list or any other of the Date formats that are applicable.

Choose the date from the calendar that the patient refused the Vaccine.

Choose the Magnifying glass to select the Historical Source.

Choose the appropriate Source from the list "01" is the typical pick stating it is the patient. (PLEASE NOTE: you must use the exact list that ships with the Aprima Product. Adding your own reasons to this list will cause errors when transmitting to the state)

You may also choose to enter a note, but it is not required to do so.

Click on OK to save your immunization to the Patient Vaccine Card.

Now the vaccine is on the Card, choose the Refusal column and choose the magnifying glass to select reason from the list.

Choose the Refusal reason. (PLEASE NOTE: you must use the exact list that ships with the Aprima Product. Adding your own reasons to this list will cause errors when transmitting to the state)

Next you will document the Completion Status. Choose the magnifying glass to select from the resulting list.

Choose the "Refused" from the resulting list. (PLEASE NOTE: you must use the exact list that ships with the Aprima Product. Adding your own reasons to this list will cause errors when transmitting to the state)

Click OK to save the vaccine and the refusal of that vaccine to the patient record. This will also queue the vaccine to be sent to the State Immunization Registry.
