Navigate to [https://kelseyportal.kelseycareadvantage.com/Application/ApplHome/Index](https://kelseyportal.kelseycareadvantage.com/Application/ApplHome/Index)

Enter your user information and click **Login**.

Click **Reports**

Click **Book of Business Report**

Make sure the Agency ID and Status ID dropdowns are filled out, then click **View Report**.

Click the Save image, then select **CSV (comma delimited)** or **Excel**
