      Check your email for the payment form \ (from [[email protected]](mailto:[email protected]); subject line: 2024 MADACS Entries - Payment Form) The file will be sent as an editable .pdf, pre-filled by MHA with your submission(s) details from the day prior and total amount due. \ \ **\*This form must be completed and sent back to [[email protected]](mailto:[email protected]) no later than 3:00 p.m. the day received in order for your entries to be accepted.**
      Review the top half of the form to ensure everything is correct. If there are errors, respond to the email from [[email protected]](mailto:[email protected]) immediately.\ \ If all is correct, complete the payment information on the bottom half of the form. The form is editable so you can type directly into the fields.
      Once you've completed all fields, sign for authorization. \ \*Digital signatures are accepted.
      Prepare to save your completed file by clicking on "Menu"
      "Save as"
      Choose a place to save on your computer, rename if you'd like, and click "Save"
      Reply to the original email, click "Attach File..." - select and send.\ \ PAYMENT FORM MUST BE RECEIVED NO LATER THAN 3:00 P.M. THE SAME DAY AS RECEIVED OR ENTRIES WILL NOT BE ACCEPTED.
      You will receive a response with "Received" if it went through. If you do not receive a response within one business day, please contact Madison at 952-548-2205.
      A receipt will be sent once the card has been processed; please allow up to two business days, depending on number of entries received.
