Go to [www.enviveonline.com](http://www.enviveonline.com) and choose "Book online" for the location you wish to schedule for.

Scroll down and choose "Schedule Online Now"

If you've never been to Envive choose "New Patient" and register an account. If you're an established patient choose "Existing Patient".

If this is your first time -- choose "Next Step" when asked if you already have an account.

To create your online account -- fill out the required information and choose "Save and Continue."

If you are scheduling a New Patient Visit, select that option.

Find the most convenient date and time for your appointment.

Confirm the appointment details and choose "Book Appointment."
