Log into your patient portal and go to the Messages tab. Click Compose New to start a new message to your Clinic and Provider.

If your Clinic or Service Site is not the Default, click the down arrow to the side of the Service Site to see a complete listing of Liberty Doctors Service Sites.

Choose your Service Site from the resulting Drop down list. If you are unsure of the name of your service site or its location, please call your providers office for assistance in which to choose.

Choose from the drop down that is most representative of type of message you are sending. Patient Attachment is for items you wish for your service site to have to attach to your patient record. **EXAMPLE:** a Blood Pressure record or immunization record.

Other options are **Appointment Questions** or **Billing Questions**.
**Demographic Questions =** changes such as a new phone # or address update you wish your provider to update in your chart.
**Lab Questions =** about lab tests or other tests which you may have received a message or update on.
**Login Assistance** for portal questions - setting up security questions for portal password resets.
**Prescription RX** requests or questions (it is recommended for refills to contact your pharmacy to send your provider a refill request)

Enter a brief Subject of your questions.

Type in your message or questions in the Message section. Once your message is complete, click on Send button at bottom right to send the message to your provider. Please allow 24-48 hours for a response from your provider or providers staff.
