Click "SEND REQUEST" You will receive a secure link that will expire in 10 minutes.

If Text message was selected, the patient will receive the text on their mobile device so have them select the blue link in the text message. If it was an email you sent, have them click on the blue link in the email.

Have patient select the highlighted link to select or drag an image.

At the pop up, select “Take Photo or Video” to access their camera to take the picture with their smartphone camera. Tip: If they already took a picture, then the image will be in their photo library on their phone and will need to be selected from that location.

Once the photo is taken, have them choose “Upload and Send Files”.

To view the image sent, Click the Worklist drop-down and select "My Assigned Items" worklist in Tracker. If you have a group worklist for Patient Request Forms, it will be on that worklist too.
