To access the Efficacy Audit via Cleaning Standards, click "Cleaning Standards"

Click "Efficacy Audits"

Click "Data Capture"

To access the Efficacy Audit via Sigma, click "Sigma"

Click "Efficacy Checklist"

If you have access to more than one hospital, select the required hospital

Select the required ward

If any questionnaires have already been completed, you will see the score, the date the questionnaire was submitted, and who submitted it

Click "Complete Now"
