For customers interested in purchasing Trip Cancellation/Interruption Insurance: **Enter** the **total trip cost divided by the number of insured individuals.** This applies only to individuals who are eligible to be covered under the same contract. 2. Specify if the customer wishes to add Pandemic Protection to their travel insurance policy by selecting **"YES**" or **"NO"**. *(Hover over the linked text for additional details.)* 3. Choose the destination from the drop-down menu. If the trip includes multiple destinations, select **"Multiple"** from the drop-down menu or choose the location where the insureds will spend the majority of their time.

    Hover over the **"Insured Person(s) List"** text to view the eligible dependents that can be included on the policy.

    1. Complete the required fields for *all* travelers. 2. To remove an individual from the quote/application, click the icon under **Action** beside the name of the person you wish to delete or remove from the application. Click **"Continue"** to proceed.

    On the **Available Products** screen, choose coverages based on the customer's specific needs and budget. 1. Travelers who are required to complete Health Declaration C will have the option to select a reduced stability period by paying an additional premium. 2. The [deductible](https://qc.bluecross.ca/partner-guide/procedures_en/deductible) refers to the amount the customer will be responsible for in event of a claim. It defaults to $0, but you can select a different deductible from the drop-down menu to fit the customer’s preference. 3. Click the "linked text" to view a pop-up with an overview of the benefits included in each available product or package. 4. Select product and customize coverage (*if appliable).* For additional details on available products and coverage, please refer to the [Partner Guide.](https://qc.bluecross.ca/partner-guide) 5. Explore additional purchase options by clicking the available drop-down menus. Once selected, the **product** and coverage options will turn **blue**, indicating they are being applied for. **Note:** Accidental Death and Dismemberment and Air Flight Accident benefits *must* be purchased as a package. Click **"continue"** to proceed through to the application.

    ### **Health Declaration C – General Instructions** 1. When you reach the Health Declaration, first check the top of the page to identify which traveler it pertains to. If applicable, additional Health Declarations for subsequent travelers will appear individually to assess each traveler’s health and generate a personalized premium. 2. Read all instructions carefully and completely before proceeding.

    ### **Health Declaration C – Section 1: Excluded Medical Conditions** This section identifies any medical exclusions and calculates a personalized premium. If a traveler answers “yes” to any of the listed conditions, the system will prompt additional, condition-specific questions. Based on the responses, certain categories of conditions may be excluded. If applicable, a medical questionnaire will be provided for the excluded condition(s).

    ### **Health Declaration C – Section 2: Personalized Premium Assessment** Section 2 of Health Declaration C does not impact or limit coverage. Instead, it evaluates personal risk based on factors commonly associated with an individual’s health and lifestyle choices. Everyday habits such as physical activity, diet and nutrition, sleep quality, and stress management are key contributors to lifestyle-related chronic conditions and are considered in this assessment.
