Requesting a Carrier Contract through the Platform

If you're an existing agent through the Spark platform, follow these steps to request a contract in the Carrier Contracts tab.

Click **Back office**.

Under Carrier contracts, select **View** **carrier** **contracts**.

Click **Request a contract**.

Choose between **English** and **Spanish**, and confirm your **Agent information** is correct. Make any necessary changes if applicable.

Confirm your **Agency** **information** and click **Continue**.

With the contracts present, choose if they are **New,** a **Transfer**, or if you want to **Ignore**. Your first 5 carrier contracts will be processed in the order listed below. Additional contracts will be processed at a later time. **Note**: Contract requests are organized into three rows: National, Regional, and Supplemental Carriers. Agents can only view Regional and Supplemental carriers after selecting the dropdown below each of these headings.

Once you've selected your contracts, confirm if you've completed AHIP certification. Click **Continue**.

Depending on the Carrier Contracts that you select, you might have to fill out additional information which would include: Background Information, E&O Insurance, Banking information, and other questions (Social Security Number, Commission Advancing).

Answer all the background questions listed.

Fill out your E&O Insurance information and then click **Continue**.

If a carrier requires your Social Security Number, input your SSN and then click **Continue**.

Please enter your banking information to make sure you receive your commissions.

Enter a Commissions beneficiary and then select **Continue.**

Review your requested Contracts. Want to make changes? Select **Edit requested carriers.**

Click here to proceed.

Click Review and Sign

You're able to download a PDF of each document.

Draw your signature and select **I accept my signature**. You can also click **reset** if you need to redo your signature.

Draw your initials and select **I accept my initials.**

Click here to officially sign all your Medicare Contract Request documents.

You can download a PDF and save it to your device.

Click **Submit contract request** to submit the request to Spark.

Spark can now begin processing your request. **Note**: There might be some actions you have to complete. After submitting, make sure to read through each action item.
