    Set up NSH Email and Network Access for a PFA

      Open a new request

      Open your web browser and go to <https://ictservices.nshealth.ca/assystnet/> or double-click the "IT Self-Service button on your NSH computer's desktop. Type in the username and password you use to log into your NSH computer and email accounts, then click "Sign In" or hit the Enter key.

      Click "Services and Support", then click "Access"

      Click "Setup User Account"

      Choose the Approver and Reason for Request

      PLEASE NOTE: Complete the request form according to the team, committee or council the PFA has joined. The Approver, Cost Centre, and Department fields should all be associated with the team the PFA has joined.

      **Approver:** Type the name of the Approver in the Approver field. Suggested approvers will appear based on what you type. Select the appropriate Approver.

      If you can't find the appropriate Approver, click the magnifying icon and use the search tool to find them.

      The **Reason for Request** depends on whether the PFA has/had an NSH account before: - **If the PFA has never had an NSH account**: choose "Volunteer" - **If the PFA had an NSH account but it is no longer active/used:** choose "Enable deactivated account".

      Fill in User Information

      **First Name, Middle Name, Last Name:** Type in the PFA's name **Phone Number:** Type in the PFA's phone number **Email Required:** Choose "Yes"

      **Have an IWK/NSH Email Address before:** If the PFA does not already have an NSH email account, choose "No" for this field. If the PFA already has an NSH email account but it was deactivated (due to inactivity, etc), choose "Yes" for this field and enter their existing NSH email address in the "**Provide email address**" box that appears.

      Fill in Position Information

      **Job Title:** Type in "Patient Family Advisor" **Department:** Type in the department associated with **the team the PFA has joined** (such as Palliative Care, Public Health, Continuing Care, etc) **Physician or Resident:** Select "No"

      **NSH/IWK Employee (paid through SAP):** Select "No" **Cost Centre:** Type in the Cost Centre number associated with the team the PFA has joined **Primary Site Facility:** In the dropdown, select the NSH site or facility closest to where the PFA lives. You can also type an NSH site name in the box and suggestions will appear matching what you type. **Secondary Site Facility:** Leave blank

      **Effective Date:** use the date picker to choose the date the PFA joined the team, committee or council. **End Date:** If you know when the PFA's term on the team, committee or council will end, use the date picker to select that date. This will shut down their email account upon that date. **Network (login/AD) Email Options:** Select "Network & Email"

      Submit request

      Leave the remaining fields blank/unchecked, read the Privacy Notice, then click "Submit" to submit the request.
