Set up your Challenge Questions

Read the instructions in the form, then type your answers to all three questions.

Type in all of your details in the boxes. Your "Primary User ID" is the ID you were given as part of your NSH email account. For the signature box, type your name and it will appear in cursive font. Type in today's date in the Date boxes.

Once you finish filling out the form, click "Save" and save the completed form on your computer. Make sure you remember where you saved the form.

Your completed form needs to be emailed to **[email protected]** . Click the email link in the Challenge & Response Form to open a new email automatically, OR open your Outlook Mail window and type the email address into the "To" field.

Type a subject and brief message.

Attach your completed Challenge & Response form by clicking "Insert", then "Attach File"

Click "Browse This PC..." and find your completed form on your computer

Once you've found your completed form, click on it once then click "Insert" to attach it to your email

Click "Send" to send your completed form to the IT Service Desk.
