Click the "I confirm that I have read and understood and agree to this declaration" field.

Click and insert the "Business IRD Number".

Click and insert the "IR Customer Name".

Click an insert the mobile number in the "Contact Mobile" field or "Contact Other Phone".

Insert the Bank details in order for you to receive your payment.
