Solis Health Plans: New Contract

You will be directed to [https://sos.solishealthplans.com/auth/signup](https://sos.solishealthplans.com/auth/signup) . Click "Sign up now".

Click "Send verification code" after entering your email. Do not enter the rest of the information until you do that.

Go back into your email, copy the code, and repaste it into the Solis webpage.

Press [[ctrl]] + [[c]]

Click "Verify code"

Fill out the rest of the information, then Click "Create"

Click "Start Registration"

Enter your SSN and proceed through the questions.

Use the navigation along the bottom to move from one section to the next.

On the page where you can upload your documents: select "browse", then select the file, then click the blue upload button to the right of "browse" to actually upload the document. Do not forget to do this or the file won't save.

Click "Save"

Click "Submit"

Click "Yes"
