Click the "Your email address" field.

Type in your ResultsHQ mobile email address.

Click the "State-Centre-Family name" field.

Type in your First Name and Last Name.

Click the "Subject" field.

Type "User Account Deletion - ResultsHQ Mobile"

Click the "Description" field.

Type in a request to delete the corresponding account. The following is a sample template you can use: "Please delete my account in the ResultsHQ mobile app associated with the email address: [[email protected]](mailto:[email protected])"
