    Navigate to [https://libguides.depauw.edu/ereserves/request_form](https://libguides.depauw.edu/ereserves/request_form)

    Make sure to fill out all relevant fields to make sure we have the information for your request (these are marked with an \*). To find your course, select the dropdown. Your course will only appear on the list if you have already requested at least ONE item for course reserves. **If your course is NOT on the list**, follow the directions in step 3. **If your course IS on the list skip to step 7.**

    If your course does NOT appear in the course list, select the highlighted words "Enter it manually instead." This will allow you to fill in information about the course for which you want Course Reserves (complete list in step 4 below).

    If your course is new, please make sure to fill out the requisite information to make sure we connect your course to the correct Moodle course. Check to see if your name is on the "Instructor" drop down.

    If your name (or any variation therein) is NOT on the Instructor dropdown, click the "Enter new instructor name here" field. And type your name "First Last" style.

    Click the "Term" dropdown and select Fall 2024 or the appropriate term for the course you are teaching.

    Click the "Start Visibility \*" field to let us know when you would like to have the item available to your students. **This is most important for films**. For many digitized films copyright only allows for a 2-week viewing visibility to students. For any other format besides films, the dates of the semester are best for this field.

    The "Type" dropdown helps us organize and retrieve your items. Please be as accurate as you are able to let us know what type of item you are requesting. This will also appear to your students.

    Please include any additional information or instructions in the box above the "Display Info" section. After reviewing your form, click "Submit". You will receive a confirmation email from "do-not-reply" with a subject line that starts with "\[LibGuides\]" The "Start Over" button will clear the form without submitting it.
