After the conclusion of your Mentorship Session, you will receive an email from Acuity prompting you complete the Mentorship Feedback Form. Click the link in the email to complete the form.
After clicking the link, you will be taken to the following form.
Enter your first name.
Enter your last name.
Enter your email address.
Click the option that best aligns with your experience attending the Mentorship Session.
Note: If you were unable to meet with your mentor and click one of the "No" options, you will not be required to complete the following questions related to your Mentorship Session. However, you are still required to submit this form.
Click the box to enter the date of your Mentorship Session.
Select the month, year, and day of your mentorship session from the dropdown menu.
Enter the name of the mentor with whom you met.
Click the dropdown menu and click the option describing how likely you are to recommend your mentor to a friend or colleague.
Click the dropdown menu and click the option describing how likely you are to recommend Startup Spring Training to a friend or colleague.
Enter any feedback you would like Startup Spring Training to provide to your mentor. Tip: Make sure your feedback is direct, specific, and concise!
