Select your office name and yourself in the drop down field

Click the "\* Patient ID" field, enter ID and DOB

Click Claims and Payments on the top Menu bar

Click the "Select a Patient" field, and select the patient you just verified coverage for. You can verify coverage for several so you can do claim after claim without having to go back and verify more.

Scroll down and Click the "Authorized Plan to Remit Payment to Provider, Select YES

Click the "Select a Provider" field. and Select yourself

Click the "Patient Control Number / Claim Numberhelp \*" field, and Enter MOM or BABY...or any random chart identifier you have or your EHR has.

Click the "Place of Service" field. and select your Place of service

Click the "Frequency Type \*" field, and Click "1 - Admit Through Discharge Claim (a)"

Click the "Provider Signature on File" field, and Click "Yes"

Click the "Release of Information\*" field, and Click "I - Informed Consent to Release Medical Information for Conditions or Diagnoses Regulated by Federal Statutes"

Click the "Provider Accepts Assignment" field, and Click "A - Assigned"
