Navigate to [customtruck.com](https://customtruck.com/)

Under the Financing tab, select **Credit Application - Equipment Sales.**

If applicable, enter your CTOS sales representative's information.

Complete the remaining fields for the business.

Click "NEXT" when all required fields are populated.

Provide ownership and/or guarantor information. If this matches the primary contact and business details, simply check the box to auto-fill the information.

Complete the remaining fields for the owner/guarantor.

If available, enter the equipment details for which you are seeking credit. (Optional)

Carefully review the document to ensure all information has been entered accurately. Once verified, sign the document using DocuSign by clicking on the "Sign Here" prompt.

Once you’ve signed the document, click “Submit.”

Your credit application has been successfully submitted! Make sure to record your confirmation number and watch for a confirmation email. You can now click "Return to the Home Page" or close your browser.
