    In the Admin Dashboard, click "Features"

    Click "Surveys"

    Click "Add New Survey"

    Enter the survey name and description

    Set a start date to control when your survey becomes accessible to members and an end date to automatically close it when you're ready to collect results.\ Completion Notification Email receives an email alert whenever someone submits your survey.

    Enter the web address (URL) where members will land after finishing the survey.

    Click this button.

    Create your survey questions by clicking the "Add New Question" button

    For questions that only accepts a single answer, choose **Multiple Choice Single**

    If you require members to answer a field before they can submit the form, this box can be enabled

    Click "Add New Option"

    Enter the option name

    After adding your answer choices, click "Continue" to proceed

    For questions where the answer is simply yes or no, choose the "Yes/No" question format

    For questions where respondents can choose more than one answer, select Multiple Choice Range
