Navigate to [https://3c2a.my.site.com/schoolportal/s/](https://3c2a.my.site.com/schoolportal/s/)

Click "FORM 3" from the header or the tile.

Click the sport that needs to be viewed.

Click "Access Student" next to the student you want to view.

This will bring up the Student-Athlete Profile view.

Last High School attended and date of attendance.

Current College start date information will be found here.

This section will populate previous history including colleges attended, athletic participation history, all experiences (aka periods of extended time away, employment, and any additional history).

Previous Colleges attended the student-athlete has self reported and will be listed here.

Previous Sports is the athletic participation history the student-athlete has self reported and will be listed here.
Alert! If a student-athlete lists "Athletic Waiver" please reach out to the student-athlete to confirm what type of waiver they have received (e.g., 3C2A injury/illness appeal, 3C2A Extenuating Circumstances Waiver, Medical Hardship waiver from another intercollegiate governing body or Covid relief.
Be sure to confirm any appeal with the correct documentation (e.g., Form 2/Tracer, Old Form 3 system, etc.).

All experiences is a summary of the below sections of the Form 1 responses for periods of extended time away, employment, unemployment, military service, church mission, and any additional history.
Alert! The Form 3 and the Form 1 does not have a rules engine to flag gaps in time between high school attendance and college attendance. Please be sure to review any gaps with the student-athlete.

Tip! Before you begin to certify a student-athletes eligibility be sure to completely review their Form 1 responses for accuracy.

**Form 1 - Official Use:** This section will be use to included information to certify a student as eligible/ineligible.

**Begin Eligibility Certification.** Click on the pencil icon in the Form 1 - Official Use area to begin to certify eligibility for the student-athlete.
New fields include Current Total Units Enrolled, # of Units passed towards meeting the 24/26, and Current GPA since first competition. These fields are **not required** to move a student forward in the certification process.

Alert! The Form 1 - Official Use section does not have built in rules, the institution is required to correctly apply the legislation for each student-athlete they are certifying.
