    Navigate to [https://3c2a.my.site.com/schoolportal/s/](https://3c2a.my.site.com/schoolportal/s/)

    Click "FORM 3" from the header or the tile.

    Click the sport that needs to be viewed.

    Click "Access Student" next to the student you want to view.

    This will bring up the Student-Athlete Profile view.

    There is other information that will be shown on the Student-Athlete profile if applicable. A flag will populate if a student-athlete indicated "yes" to the question related to amateurism status. If a student-athlete opts out of allowing the institution and 3C2A to use their Name, Image, and Likeness (NIL) there will be a note. **Please see examples of both below.**

    An example of a student-athlete who has NOT authorized the use of their NIL will show here.

    An example of a student-athlete who answered "Yes" to the following question related to their amateurism status will show here. ***"Have you competed in this sport professionally, taken pay in any form for participating in this sport or signed an agreement to compete professionally in this sport?"***

    Alert! For institutions who opted to include the Felony Disclosure Form as part of their online Form 1 workflow a Flag will populate (see example below) and will allow the institution to connect with the student-athlete to inquire more about their selection on the form.

    Tip! While reviewing the student-athletes responses if the institution discovers an error that requires correcting or an update the institution can make those changes from the Student-Athlete Profile page.
