    **Navigate to --&gt;** <https://app.topsheet.io/dashboard>
    This is just an example of State Tax forms, but is a similar process for all needed start forms such as I9's, W4's, and Bank Information (for payment)
    From your dashboard Click "**ACCOUNT**" The ***ACCOUNT*** button will be at the Top Right Corner.

    Payment & Bank Information

    Click "**Banking Information**" , here you will set your Account and Routing information so that we can pay you quickly and simply.
    Fill out the **Account Nick Name**, **Routing Number**, **Account Number**, and **select the account type** such as Checking or Savings.
    Tip! You can Add and Agent account if you personally pay your agent a % of each of your paychecks, if production is paying your agent for you, do not use this. You can also use this for Coogan Accounts to allocate the required 15% to the Savings Account for Minors.
    Click "**Submit Changes**" when done.

    Finding Start Forms

    Click "**My Documents**" to find your start forms.

    I-9

    Click "**I9**" to get started on your documents for work status in the US.
    Follow the instructions, and **fill out each item** with the information that would be used on documents such as driver's license or passports, as this goes directly to the Social Security Administration (SSA) for approval, and they regularly with flag anything that doesn't exactly match. IF that happens your production team should be able to help you.
    **Select your legal status**.
    Select if you used a translator to fill this out.
    When you have finished by selecting your signature then click "**NEXT: UPLOAD I-9 DOCUMENTS**"
    **I-9 DOCUMENT OPTIONS:** **LIST A**: You will only need to upload 1 document. it can be any of the following. - **Passport of the U.S.**: A U.S. passport or U.S. passport card. - **Permanent Resident Card or Alien Registration Receipt Card (Form I-551)**: Commonly known as a "Green Card." - **Foreign passport** that contains a temporary I-551 stamp or temporary I-551 printed notation on a machine-readable immigrant visa. - **Employment Authorization Document (EAD)** that contains a photograph (Form I-766). - **Passport from the Federated States of Micronesia (FSM) or the Republic of the Marshall Islands (RMI)** with Form I-94 or Form I-94A indicating nonimmigrant admission under the Compact of Free Association Between the United States and the FSM or RMI. - **Foreign passport with a Form I-94 or Form I-94A Arrival/Departure Record**, bearing the same name as the passport and containing an endorsement of the alien's nonimmigrant status, if that status authorizes the alien to work for the employer. - **In some cases, a passport from a foreign country with a Form I-20 or DS-2019 and a Form I-94 or I-94A**. This is applicable for nonimmigrant F-1 or J-1 status holders who are authorized to work for a specific employer as part of their status (students in practical training and exchange visitors, respectively). **LIST B**: You will need to upload a List C document as well, but List B can be any of the following. - **Driver's license or ID card** issued by a U.S. state or outlying possession of the U.S. provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address - **ID card issued by federal, state, or local government agencies or entities**, provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address - **School ID card with a photograph** - **Voter’s registration card** - **U.S. Military card or draft record** - **Military dependent’s ID card** - **U.S. Coast Guard Merchant Mariner Card** - **Native American tribal document** - **Driver's license issued by a Canadian government authority** For employees under 18, documents such as a school record or report card, clinic, doctor, or hospital record, or a daycare or nursery school record can also be accepted under List B. **LIST C**: You will need to upload a List B document as well, but List C can be any of the following. - **Social Security card** issued by the Social Security Administration (other than a card stating it is not valid for employment) - **Certification of Birth Abroad** issued by the Department of State (Form FS-545) - **Certification of Report of Birth** issued by the Department of State (Form DS-1350) - **Original or certified copy of birth certificate** issued by a state, county, municipal authority, or territory of the U.S. bearing an official seal - **Native American tribal document** - **U.S. Citizen ID Card** (Form I-197) - **Identification Card for Use of Resident Citizen in the United States** (Form I-179) - **Employment authorization document issued by the Department of Homeland Security** These documents are used to confirm that an employee has the legal right to work in the United States. Employers must record the document title, issuing authority, document number, and expiration date, if any, in Section 2 of Form I-9.
