To do a Stand alone Fall risk Screen in conjunction with any type of office visit, go to Health Maintenance and add the Fall Risk Screen in your CC tab.

Add either the STEADI Fall risk Assessment or the Timed Get Up Assessment to screen the patient for the fall risk.

Go to HPI and Select the Fall Risk Screening.  Answer the applicable questions.  Selecting the STEADI Assessment Score: will add Code 3288F
