Alert! This document covers **ONLY** the parts of the Medicare Annual wellness visit which **automatically** drops billing codes for the user. ALL sections of the AWV should be filled out for a Medicare Annual Wellness visit.

Choose System Health Maintenance then Medicare Annual Wellness Visit

Add any applicable Assessments for the visit, which can include GAD-7, PHQ-2/9, UDI-6, MMSE, FAST, Portable Mental Status, CDR, HHIE-S, CAGE-AID, STEADI AUDIT-C- (does drop procedure G0442 when added as an assessment), AUDIT-10- (does drop procedure G0442 when added as an assessment)Add each applicable form needed.
NOTE: Only the AUDIT for alcohol will Automatically add a G0442 code to the SP tab. (not shown)

Go to the HPI tab. **The next slides will only show sections that automatically drop DX or SP codes automatically**. All items on the Category list should be addressed and documented for a visit.
**Visit Type-** Select one of the options for Welcome = G0402, Initial= G0438 or Subsequent= G0439 this example shows subsequent being selected.

**Depression Screening PHQ-9** - if the visit is for a SUBSEQUENT visit, choose the Bill depression screen subsequent visit to automatically drop the billing code G0444. Select the applicable score that was shown on the PHQ-2/9 done on the Assessment tab. Based on the score either G8510 for negative screen 0-4, or G8431 for Positive >5 will be added.

IF the Depression Screen was Positive, Score >5, then also include the +Depression Follow up plan. If Negative score, skip the + Depression.

Choose the **+ Depression Follow up Plan** from the list, or choose the Other\_ to enter your custom plan if not listed.

On +Depression plans, you may receive the message below. This is normal, it is letting you know that the G0444 code is already on your SP tab and will not be added a second time. Click on OK to proceed.

**ADL's** - When selecting any of the ADL's Independent or Patient is NOT independent the 1170F will automatically drop to the SP tab

**Advance Care Planning** - If the AWV is a Subsequent Visit, click on the Bill Subsequent Visit Advance Care Plan to drop SP code of 99497

**Opioid Use** - Pain scale in this section will drop codes 1125F or 1126F based on the patient Pain value 0-3 = 1126F where >4 = 1125F

Choose the applicable 0-3 or the 4-10 range and at the \_ enter the value the patient states is there current pain level.
