Open the patient to the demographics page and click on New to start a new outreach call note.

Choose the New Outreach Call from the drop down list.

IF APPLICABLE (patient is not associated with a provider in demographics)

IF APPLICABLE, choose the provider associated to this patient -who they would follow up with.

IF APPLICABLE click OK to save this provider as the rendering provider

On the CC tab, select "Other" from the selections on the left.

Choose the "Transition of Care Outreach" from the List on the right.

Go to the HPI tab.

Answer all questions that are on the Left side by choosing the columns on the right. Enter the discharge facility name, Discharge date etc.

Example: Type in the Facility that the patient was discharged from, then continue to answer each question as applicable that the patient answers.

Note here, if this is first contact, choose the Initial contact, but if 2nd or 3rd contact choose applicable Contact dates. You may use this same note if you have attempted multiple contacts by editing this note each time you contact the patient.

Continue answering questions as applicable until all questions are answered.

If the patient is to be scheduled for a future follow up, enter the date here and insure that the patient is placed on the Providers schedule for the future visit date.

You may also wish to enter a DX for this patient if one is known on the DX tab. once DX if entered is done, click ok to close and save this note.

If this is a completed Outreach call, choose the Complete Note. (may not be completed if it is an initial or 2nd call and no contact was made, for these leave the note Incomplete so you can use same note for additional outreach contacts)

Click OK to Save your Incomplete or Completed note

Click OK on the Patient Provider Summary window. This note is now under the patient Review Past notes to access or view and to be assessable to pull into the Outreach Visit note (See Transfer of Care Visit Note for those instructions)
