      Section A - Plan Information: Enter the RDSP Account Number which is available through your Advisor Portal. Enter the full name(s) of the Account Holder and Joint Account Holder (if applicable). Enter the Beneficiary's full name.

      Section B - Contributor Information: Enter the Name of the Contributor (a non-Account Holder) - this is the person that will be making deposits into the account and is NOT a Holder. Include their living address. You also have to state their employment. If unemployed, please type 'Unemployed' or 'Retired' (if retired). Their Date-of-Birth must entered. You must also include this person's relationship to both the Account Holder and the Beneficiary. Check the box that represents if this person intends to make a one-time Contribution and/or regular Contributions.
