Navigate to [my.stevens.edu](https://login.stevens.edu/app/UserHome?iss=https%3A%2F%2Flogin.stevens.edu&session_hint=AUTHENTICATED)

Click on the "Healthy Stevens Portal" tile.

Enter your date of birth in the format MM/DD/YYYY.

Click "Requirements & Forms Not Satisfied"

Locate the required immunization and click "Update"

Enter the date in the "Date" field in MM/DD/YYYY format and select the type of immunization.

To upload supporting documentation for your immunizations, locate "Immunization Records" and click "Update"

Locate your file and click "Upload". You may experience difficulties with an upload if the file name contains special characters or if the file type is something other that .jpg; .png; or PDF.
