Access On Mobile

Install Hale Hub App 1. Open the App Store on Android or iOS. 2. Search for "Hale Hub." Hale Hub by ModuleMD LLC should show up in the list. 3. Follow the standard procedure for your phone to download the app. 4. Login using the credentials from your Portal Registration email.
Access on Computer

1. Navigate to <https://mmdpas.modulemd.com/PatientPortal/login> 2. Enter Username & Password 3. Click "LOGIN"
Select the Patient

If multiple patients are associated with the email address, select the correct patient from the list.
Update Demographics

Click here.

Click "Demographics"

Not all demographic information is visible on the summary page. **Click "Edit" to review & update.**

1. Update all the requested information, then 2. Click "Next"

Update your Insurance Information and click "Update."

This box will show that you've successfully updated your demographic information.
**Note** - You will not see the changes until we've had a chance to review and accept them.
Complete Intake Paperwork

Click "Patient Forms"

**Complete all of these forms BEFORE your appointment.** If they are not completed BEFORE your appointment time, your appointment is subject to being rescheduled.
