Accessing Club Sports Agreement to Participate Form for 2025-2026

All members of club sports are REQUIRED to complete the Club Sports Agreement to Participate (2025-2026) Form found on each Club’s individual UVM Club’s page. This should be completed prior to each individual’s first practice with the club. Individuals who refuse or do not sign the agreement will not be eligible to participate in Club Sports activities.
