Navigate to<https://myoiss.yale.edu/>

Click 'Don't have an institutional login? Use a one-time passcode.'

Enter your email address and click 'Request Login Code'.

Paste the login code from your email. It will validate and give you a success message.

Click the 'Documents' tab on the left sidebar.

The 'Documents' tab shows a list of all your current immigration documents in the system. Once OISS creates and signs your DS-2019, it will appear here for you to download.

Download the file by clicking the down arrow to the left of the document.
