Accessing the Student Medical Information Submission Form through Skyward

Sign into your Skyward family access account.

Locate the student you want to submit medical information for, and select the **CHSD 230 Student Medical Information** tile.

Review the instructions and information on the first page, then click **Start** at the top.

Complete the **Emergency Information Form**, **Health Forms**, and **Helpful Links** steps. After completing each step, click **Complete & Next** on the top.

Review the terms of agreement, and select the **I Agree** checkbox to provide your electronic signature.

When finished, click **Submit Form** at the top.
