Add Members and Users in the Portal System

Navigate to <https://map.ddkygroups.com/MAP>
Find a Member

Click "Search for a Member"

Click the "Last name and date of birth" field.

Click "Search"

Click "Select" to select a member.

Here's where you can view the member's benefit information. Click the tab labeled "DeltaVision" to view vision benefit information.
Add a Subscriber

Click "Add Subscriber"

Click the "Search Group" field.

Select the benefit plan for the subscriber.

In the "Qualifying Event" dropdown, select "New Coverage". **Please ignore the "DDKY Use Only" option.**

In the "Hire/Event Date" field, type in the desired effective date.

Fill out all applicable information to the subscriber.

Social security number is required.

Marital status is required, regardless of age. If you do not wish to provide this information, select "Not Provided".

Fill in subscriber's contact information.

Verify information is correct and click "Add" again.

If adding more than one benefit plan (i.e., dental and vision), click "Add New Enrollment" on the Enrollment Summary page that appears after you add a subscriber.

If a subscriber is changing from one dental plan to a different dental plan or transferring to a different location, they must be termed under the first premium group and added as a new subscriber under the other premium group.
Add Dependent

Select your relationship to the dependent (child, spouse, etc.)

In the "Hire/Event Date" field, type in the desired effective date.

Please **do not** add social security number for dependents (including spouses).

Fill in dependent's contact information.

Click this checkbox if the dependent has special needs that require services beyond the group's maximum dependent age.

Term a Subscriber or Dependent

Search for dependent using member search.

On the right side of the member you wish to term, click "More"

Click "Terminate Policy"

Select "Other" as the termination reason. Type in the date of the last day in which the member should have coverage.

Click "Terminate"
View Benefit Summary

In "Document Type", select "Summary Plan Description".

Add New User

You will create a username and password for your new user. You will be responsible for relaying the log-in information (username and password) to your new user.

Edit Access & Finance = Permission for full edit and all invoices\ Edit Access = Permission for full edit access but cannot access invoices\ View Access & Finance = Permission to view members and invoices\ View Access = Permission to view members but cannot access invoices

Click "Create". Advise your new user to change their password after their initial log-in.
