Adding a Referral / Resource Connection to an Assessment

In this tutorial, you'll learn how to add a referral / resource connection to a family assessment Note: Not all assessments have a referral section.

On the Assessments page, find and click the "edit" pencil to open up the assessment you wish to add a referral to.

Scroll to the bottom of the record. For the question about Resource Connections, click "Yes" to open the record.

Complete the referral / connection record. Who does the referral pertain to?

The Connection Type and Connected By fields will have a default selection that can be changed.

Enter the Agency Name the family was referred to. And enter notes about why the referral is being made in Connection Reason.

Click "Save"

Click "Assessments" to return the main assessments page.

To update the referral, you can either go back to the assessment record where the referral was entered, or you can use the Family Assessment Resource Connections table below it to locate the referral you want to update. Click the edit pencil.

Click "Add new status"

Click the calendar to enter the date you followed up with the family.

Select the date.

Select the update status.

Click the calendar for the "As of" date,

Select the date to indicate when the new status started.

Click "Create" to save the new status.

Close the record.
