Click the "Username" field.

Click the "Password" field.

Click "Learn More & Submit"

Read full HEAL program guidelines including tips

Click "Apply to HEAL"

Fill in Sponsoring Organization name (example, Atchison, City of)

Click the "*Organization and Designated Signee Contact Lookup" field.

Type in Designated signee name - (This would be the person that would be listed on the contract if awarded. )

Fill in the "Designated Signee Phone Number" field.

Fill in the "Designated Signee Email" field.

Click "Save"

Ensure you are on the "Application Workspace" tab

Make sure you have selected the correct designated signee from the dropdown

Make sure this account is the correct applicant account. If not, you will need to find another account to apply for the grant.

Fill in the "\*Designated Signee Phone Number" field.

Fill in the "\*Designated Signee Email" field.

Click "Save"

Fill in the legal address of the applying organization

Fill in the "\*City" field.

Fill in the "\*State" field.

Fill in the "\*Zip Code" field.

Fill in the "\*County" field.

Fill in the "\*Country" field.

Fill in the "EIN" field.

Fill in the "UEI (Unique Entity Identifier that is 12 characters long)" field (if known).

Click the "\*Organization Type" field, and select the correct drop down organization that fits the applying organization's category.

Have you ever applied for HEAL before? What about a CDBG Commercial Rehab project? This helps us collaborate on awards and timelines.

Fill in the "\*Is this project connected to a BASE, SEED, Rural Champion, or CDBG Commercial Rehab project in any way? If Yes, explain." field.

Select if you are applying for a building grant (interior and exterior project), or Facade (only the front or street facing sides of a building).

Fill in the "\*Amount Requested for Building Grant" field.

Click "Save"

Fill in the "\*Project Name - Format (City, Address)" field.

Fill in the "\*Project Site Address (Street Address)" field.

Fill in the "\*City" field.

Fill in the "\*Zip Code" field.

Fill in the "\*State" field.

Select the size of the city where this project is located.

Fill in the "County Search" field, then select the county that matches your search.

Fill in the "\*Building area square footage or facade area square footage" field.

Fill in the "\*Building Owner's Full Name" field.

Fill in the "Building Owner's Organization Name (if applicable)" field.

Fill in the "\*Building Owner's Phone Number" field.

Fill in the "\*Building Owner's email" field.

Fill in the "\*Building Owner's Home Address" field.

Fill in the "\*Building Owner's City" field.

Fill in the "\*Building Owner's State" field.

Fill in the "\*Building Owner's Zip" field.

Fill in the "\*Building Owner's Country" field.

How long has the applying building owner owned the building? Select one.

Is the building listed on the National or State Register or in a Historic District?

Fill in the "\*If yes, please include the link to the KHRI listing." field after following the links to complete the search for the property on the KHRI website below.

Select the "Leavenworth" option.

Click this text field.

Select the building that applies to the project.

Click "Create Link"

Click "<https://khri.kansasgis.org/index.cfm?in=103-3020-00411>" copy the link, and then paste it back in the browser page of the HEAL application.

Paste the link here: "\*If yes, please include the link to the KHRI listing." field.

Fill in the "\*Describe the current condition of the building. (5,000 Character Limit)" field.

Click "Save"

Fill in the "\*Project Description and Scope of Work" field.

Fill in the "\*Explain the historical or architectural significance of the building to the downtown district." field.

Fill in the "\*Describe how the building will be revitalized and put to use." field.

Fill in the "\*Project Funding Need" field.

Click "Save"

Fill in the "\*Proposed Timeline" field.

You can expand this box to see your full text by dragging the 3 lines at the bottom right of the text box.

Fill in the "\*Project Readiness" field.

Type "Insert description outlining how you will be ready in the required timeline. Can you get this done in time?"

Click "Save"

Back to the main "HEAL 5.0" page

Click "Get Started" on the Budget line.

Click "New" to add a single line item on the budget. You will do this for every funding source, and for every expense line item to build your budget.

Fill in the "\*Funding Source" field - this is where the $$ is coming from.

Click the "\*Match Type" field and select the appropriate drop down that matches what kind of revenue source you are funding this project from.

Fill in the "\*Describe Funding Source" field with any description needed

Fill in the "\*Amount" field.

Fill the "\*HEAL Request Amount" field.

Click "Save"

Click "New" to add additional revenue line items. You will do this for EVERY type of revenue that will go into this project, EVEN if it isn't being used for match for HEAL. For example, if you have a $1 Million project, you will still show $1 Million in revenue for this section, and $1 Million in expenses. You will only fill in the HEAL Match box for the revenue that will match HEAL.

Fill in the "\*HEAL Request Amount" field only when you are using the match you just filled in to be a match for HEAL. Then include how much of this match will be for HEAL by filling in this box.

Click "Next"

Click "You will fill out each budget expense separately by clicking "NEW" below."

Click "New"

Select the "\*Expense Type" field.

Click the category of expense in your project

Click the "*Describe Expense" field.

Click the "\*Match Type" field. This should correspond to the revenue side you just completed.

Fill in the "\*Amount" field for the cost of the expense. This can come from your attached contractor estimates, architect, etc.

Fill in the "\*HEAL Request Amount" field if HEAL is covering this expense. Otherwise put $0

Click "Save"

Keep adding individual expenses to make your budget complete and accurate. Expenses side of the budget should ultimately match the revenue side you just completed.

Click the "*Expense Type" field.

Click "Plumbing and fixtures"

Click the "*Describe Expense" field.

Click the "*Match Type" field.
