Complete the registration form and click the "Continue" button.

Click the "REQUEST THERAPY ➔" button.

Click "Myself" or "A Dependent" to select the person you are seeking services for.

Complete each filed in the application.

Select the scholarship inclusion.

Read through and check each scholarship agreement box.

Print and sign your name.

Click "Continue" to submit your application. Once submitted, Building Hope staff will review your application and contact you.
