Patient will "SIGN IN" to their account

The home page display includes a tile for "Symptoms"

Patients will see a list of all of the symptoms that have been presented to them for their response

Any symptom tile may be expanded for viewing their individual symptom entry history

"MORE" or "LESS" buttons are presented to patient if the submission history is long enough

Tip! The patient must respond to the question sets at least three times to view a trend graph.
