Click "Click here to register online"

Click the consent form (top for an adult, bottom if for a child under 18), download it and read it.

Enter your email address to receive a copy of your consent form

Click "Submit"

Enter the full name of the person being screened

Click on the family member who has type 1 diabetes

Enter the age of the person being screened

If the person being screened is a child, enter the name of the person providing consent on their behalf.

Click "I confirm I am the child's parent or legal guardian"

Click "Add signature"

Sign as best you can

Remember to save your signature

Click "I certify that all of my information in the document above is correct. I understand that clicking 'Submit' will electronically sign the form and that signing this form electronically is the equivalent of signing a physical document." field.

Click "Submit"

Enter the best contact details to receive your test kit and / or test results

Enter the date of birth for the person being screened

Select the correct gender of the person being screened

Tell us if there is any other information we need to know

Select how you would like the testing to be done

Click "Submit" and then "Close survey". Congratulations, you have consented and registered for Type1Screen. You do need to do this for each person to be screened. Thank you for supporting type 1 diabetes research. Depending on how busy we are, test kits can take some weeks to arrive and results can take up to 10 weeks to come back to you. Please be patient with us. We are a not for profit research group.
