Find the ISBE 77-21A Modified Form for the current term. Click the corresponding red box labeled "Edit" on the right side.

Complete the form. Some field are prepopulated with the information we have on file. Additional required fields are marked with a pencil and red asterisk.

Click to either "Save Draft" or to "SUBMIT".
Tip! Please note that submitting this form is a prerequisite to receive the 77-21B form which contains evidence of your PD hours. If you receive an error message when submitting, please review the form for missing required information.

Once you click "SUBMIT", your 77-21B Evidence of Professional Development certificate will be emailed to the address we have on file.
