Completing the Trademarks Approval Form for Annual Registration

This guide details the Trademarks approval process for organizations who are seeking to obtain approval for an organization name and/or profile image that contains university trademarks.

Organizations wishing to use UGA Trademarks in their organization's name or profile image must request approval annually from the UGA Trademarks office as a part of registration. This guide details how to complete the Trademarks Approval form for approval of your organization's name/profile image. While the Request Form is the same, this guide **is not** applicable to organizations requesting use of Trademarks for other promotional purposes, such as merchandise or flyers. In addition to completion of the Trademarks and Promotion training module, it is highly recommended that you review the [Trademarks webpage](https://brand.uga.edu/trademarks/) to review the scope of trademarked phrases and imagery.

Are you registering a new organization or unfreezing an organization? If so, Trademarks approval for the organization's name and profile image can only be obtained once all other registration requirements are met. Please do not complete the Trademarks Approval Request Form until you receive an email from [ugaels@uga.edu](mailto:ugaels@uga.edu) instructing you to do so. Registered student organizations **do not** have access to utilize UGA Athletic Trademarks, such as Dawgs, Bulldogs, bulldog imagery, etc., with the exception of Club Sports advised by the Department of Recreational Sports.

Navigate to the Trademarks Approval Request Form:<https://form.asana.com/?k=bDIMYHF6D8znS0xH8Ry0hQ&d=59533422360744>

In the "Name" field, list your first and last name.

In the "Title" field, list your title within the student organization (i.e. President, Merchandising Chair, etc.).

In the "Email Address" field, list your UGA email address.

In the "Phone Number" field, list your phone number.

In the "UGA Affiliation" dropdown, select the student organization's corresponding affiliation.

**"Student Group - General Interest (including Intramural Sports)"** = most registered student organizations **"University-Wide Governance, Academic, and Honorary Organizations"** = your student organization has a logo within the Visual Identity System **"Student Group - Professional, Trade, Ambassador"** = advised by an academic college or university department **"Student Group - Club Sports"** = Club Sports advised by Recreational Sports

In the "Student Group Name" field, type the organization's name exactly as it would appear in the Involvement Network.

In the "Description of Items to be Purchased" field, because this guide is focused on the purposes of registration, list N/A.

In the "Business Purpose" field, list "Annual Registration".

The "Quantity to be Purchased" field can be left blank for the purposes of registration.

In the "Internal/Non-profit or Retail/Profit Use" dropdown, because this guide is focused on the purposes of registration, select "Internal/Non Profit".

The "External Sponsors" field can be left blank for the purposes of registration.

In the "Vendor Name" field, because this guide is focused on the purposes of registration, list "N/A".

In the "Vendor Licensee Status" dropdown, you can select "Exempt". The status selected should not affect your submission, given that a vendor is not used within this approval form submission.

In the "Artwork" section, upload a file of the organization's profile image.

Even if you are only requesting approval for UGA trademarks in an organization's official name (and the profile image does not contain UGA trademarks), because the field is required, you should still upload the organization's profile image.

The "Comments" field is optional if you have any additional notes for the review process.

Click the blue "Submit" button.

Once the Trademarks Approval Request Form is submitted, you should receive an email notification within 2-3 business days. Upon receiving a signed approval from Trademarks, please email the signed form to [ugaels@uga.edu](mailto:ugaels@uga.edu) to finalize your student organization's registration.
