Create a Form on the Involvement Network

Creating a Form

Navigate to the Involvement Network at <https://uga.campuslabs.com/engage/>. Log in using your UGA MyID and password.

Open the main menu on the left hand side of the screen by clicking the icon in the upper left hand corner.

Find the organization that you would like to submit an event on behalf of, located under the "My Organizations" section of the menu. Click on the gear icon to open your organization's toolbox.

From the toolbox, select "Forms."

If you are unable to find the organization, or if you do not have access to the appropriate tools in the toolbox, please contact your organization's primary contact. They will need to give you permission to access these tools within the organization's roster. They can use the "Roster Management" guide found on our website for further support with this step.

In the top right corner, click on the three dots.

Click on "Create Form".

Form Properties

In the "Form Name" field, enter the name of the form.

Form Status Enter in the timeframe when the form will be active. Toggle the "Enabled" option in the right corner to make the form available within the listed timeframe.

Include in Explore Forms List If you would like your form to be listed in the Explore Forms list, toggle the "Enabled" option in the right corner. Enabling this option adds your form to the main Forms section of the Involvement Network. The "Feature" option places the form towards the top of this list.

Require Review and Approval Process If you are interested in adding and approval process to your form, toggle the "Enabled" option. Enabling this option allows you to assign reviewers to approve, deny, and give feedback to received submissions. Otherwise, all submissions are labeled as "Received" with no additional action required.

The Review and Approval Process option may be helpful to toggle in cases such as collecting applications, where form content necessitates a review by individuals, or submitting requests, where feedback can be given before approved with an opportunity to re-submit.

Allow Submissions from Public Users If you would like the option for individuals to complete the form without having to log in to the Involvement Network, toggle the "Enabled" option.

If submissions are allowed for public users, note that multiple submissions are automatically enabled.

Submitter Identified Reviewers

Click on the blue "Save" button in the top right corner.

Editing Questions

Click on "Edit Questions" in the top menu.
