Create a response form

Create a response form, which can be used to evidence self-evaluation responses or as part of quality assurance processes

Evaluatemyschool

Open your self-evaluation

Click "Evaluation Setup"

Click "Edit Response Forms"

Click the blank text field and name your response form.

Click "Add Form"

Click on the response form to open it

Click "Add New Question"

Click the "Question" field and type your question

If required, add a description

Select the field type (e.g. long text)

If required, select a connected statement

Click "Save Question"

When the response form is ready to share with participants, click "Open"

The "Respond" tab will now be visible to participants

To delete a response form, click "Table"

Click the bin icon to delete the corresponding response form

To export response forms, click the "Export & Reports" icon

Click "Export"

Click "Responses"

Click "Generate Report"

Click "My Reports"

Click the file name to download it
