Creating Form Templates

Creating form templates is the next step after creating fields, dictionaries, and domains. Once created, a template can be used repeatedly in multiple studies and modified as needed.

To create form templates make sure you're on the Network level of your Hyggio account.

Open "Management" in the bottom left corner.

Select "Library"

Click "Form templates" tab

To create new form template select "+" icon in the top right corner.

Fill in "Name"

Select "Blinding" from the drop-down list.

Select "Performer" from the drop-down list.

Click "Done"

Select "Validation"

To add fields to the template, click "Add"

On the right-hand side, you will find all the domains with the fields you have created. Fields that have not been added to any domain are located in "Other".

Click on the name of the domain whose fields you wish to use in the template.

Each added field you can copy, delete and edit

When editing a field, you can create a conditional field that depends on another field, change the field width, the performer, blinding or the validation for that specific field. To create a condition, click on the arrow next to Conditional fields.

Select the value that will meet the condition.

Click "Done"

Click on the arrow to return to editing field elements.

You can also duplicate the field. If you tick this checkbox, it will allow you to dynamically duplicate the field during the visit.

When you go to the very bottom of the domains you will have Adverse Events, Concomitant Medication, Medical History and Attached files. These are dynamic table templates and buttons that you can attach to forms - they will allow you to add AEs, Concomitant Medications, Medical History and Attached files directly from the visit. This means that you do not need to prepare templates for these elements, but can simply use the ready-made ones included in Hyggio.

Click "Save"

You can continue working with each template you created. Clicking on the three dots next to the template will bring up several options.

You can "Edit" and apply changes, "Clone" and Deactivate. Select the one you need.
