Creating a Lock Door Restraint Report on AIMS

Navigate to AIMS at <https://www.mybsc.ca/aims_clwindsor/default.aspx>

Click "Incidents" Or you can click the number in the SAFE System under "Restraints"

Click "Yes" to create a restraint report - when a physical restraint, chemical restraint (PRN), or lock door has been used.\ \ Click "No" to create an incident report - more on that in another video.

Click "Add New" to create a new report.

You need to attach the report to a person supported, type part or all of their name here and click "search"

Select the person from the dropdown menu.

Select the Service where the Restraint occurred -for most people this will be their home - even if it happens outside their home, for this dropdown select their home or the location they receive support.

Select the date and time (please use military time) of the restraint here.

Select the first person to witness the incident and the location of the incident in this section. In the Description of Incident area please enter fact based details about what happened. This does not need to be long, but someone reading it should understand what happened.

The "Descriptions" dropdown is what makes the report a Restraint Report -- or an Incident Report, or a Med Error, or a Seizure Report. It all depends what you choose here -- do not forget to click Add Description.

Select the type of Intervention used, in this case it was a CTO/ETO - which means a Lock Door as part of an Enhanced Support Strategy.

Here is where you will add your Lock Door Monitoring Times - the frequency is in the Enhanced Support Strategy and must be followed. Enter the monitoring time, the name of the staff entering the monitoring time, and your observations at that time. Then click Add.

Use this section to indicate any de-escalation techniques attempted

Please indicate any antecedents or precipitating factors that might have led to the person requiring Lock Door.

Once you are done on this page please click Save/Next

Complete the verbal escalation continuum - what did the person supported do, and what did you do to support them. Please include N/A for any spaces that did not apply -- do not leave blanks.

Unless the person's plan states to not debrief with them, you MUST debrief with them and document it here

You MUST complete a post incident review with any co-workers that were present.\ \ Check Yes, you are the Post Incident Debriefer, it must take place the day of the incident, and you must discuss what happened and action taken - and any strategies for the future

If there were any witnesses to the Lock Door Protocol (other people supported or visitors) complete a debrief with them as well and document it here.

When you are done, click Save/Next to go on to the next page.

Click here to Sign Off and your report is complete -- unless you still have more Monitoring Times to input.
