Navigate to <https://swau.evaluationkit.com/MyEval/Login.aspx>

Enter your credentials and select **Login** to proceed.

Click "Surveys & Projects"

Click "Create New Survey"

Click the "Title" field. (Giving Your Survey a Title)

Select the "Single Selection" option.

Please enter a rating from **1 to 5**, where **1** is the worst and **5** is the best. (Poor, Not Good, Okay, Good, Excellent)

Repeat this process for the total number of questions you want to provide.

After adding all your questions. Click Save

Click the "Title" field and give your Survey a title (This is what respondents will see)

Click in the Start Date

Select the date and time when the survey should open.

Click "Survey Submit Confirmation Box Text"

Enter a message you want respondents to see when they submit the survey.

Click the "Make this survey available on a public web page" field.

Click the "Approximately how many respondents do you want to allow? \ \ You can set to how many you're expecting.

As you can see, it provides a link (the first option) and a QR code that you can either copy or download. Once you've done that, click **Save**.
