Creating a Survey in VU Collaborate

Log in to [VU Collaborate](https://vucollaborate.vu.edu.au/d2l/home), and navigate to your unit space.\ On the navbar, select **"Space Admin"**.

Then click on **"Surveys"**

Select **"New Survey"**

Provide a Name for the survey. \ *Example: Feedback Survey*

Select **"Add/Edit Questions"**

Click **"New"** and select the type of question you would like to add. \ Most commonly used questions are **Likert**, **Multiple Choice** and **Long Answer** Questions.

Continue adding questions, when complete select "Done Editing Questions" to return to the **Edit Survey - Feedback Survey** page.

Review your survey, when completed click **"Save and Close"**.

To make the survey available, go to the module where you would like to add your survey, and click **"Add Activities"**

Then click **"Surveys"**

Select the survey you would like added.\ *For example: Feedback Survey*

By default, the survey is hidden from students. To make visible, click the **hidden** icon

Then click again to toggle from **Hidden** to **Visible**
