On the client's profile page, click the dropdown arrow next to **Edit contact.**

Click **Send needs assessment**.

Choose between English and Spanish

Click **Create link for SMS / chat**.

Click **Customize needs assessment** if you would like to customize what's sent to your contact.

Click **Create needs assessment link** after you've made your choices.

Click the URL to copy it to your clipboard.

You can now copy and paste the Needs Assessment link into the body of an email. You can also use your mobile device to send the link as well.

Upon opening the survey, your contact will be able to confirm the items they want to review and then continue as expected.
