Navigate to [https://app.calstates4.com/csuchico](https://app.calstates4.com/csuchico)

Click "Student/Faculty Log In"

Login as it allows, and use the 'Forgot Password?' option as needed

Click "Manage your courses"

Click "View course" for the timeframe you are trying to obtain your evaluations from.

Click on your name.

Look for 'Student Signature' with a December date & a April/May date and "View Your Submission"

Click "View full submission"

Click "Download PDF"

Optional: Opt to Print Document from this view and choose Print to PDF
