Finding and Enrolling a patient in Carevive, PROmpt®
Patients can be enrolled in Carevive PROmpt® for ePRO services by any care team member.

Tip! Save the practice domain to your bookmark favorites for easy access

Care team member signs into their carevive domain OR clicks directly into patient dashboard if embedded in EMR.

Enter the patient's information in at least one field (e.g., "Last Name").

Click "Search"

You will see all patients that meet the search criteria. Find your patient and click "Click to Enroll"

Click this dropdown.

Select a **Primary Reviewer** from the list.

**Assign a Compliance Reviewer.** Click the drop-down or check the "Same as primary reviewer" box.
Alert! For delivery of an enrollment link to a patient, their mobile device number or email must be in Carevive

Click "Start"
Tip! Defaults for ePRO surveys and reviewers are configured for practices and locations within the settings prior to Go-Live. These can be adjusted at an individual patient's enrollment as needed.

**Patient clicks on text or email link for PROmpt Welcome Sign-Up -** Patient will click play to watch a brief ePRO education video

Patient confirms participation with "Yes"

Click "SUBMIT"

Patient selects their communication preference(s)
Tip! Most patients choose to use PROmpt via their mobile device, but both contact options are recommended for sign-up.

The patient will create a username for PROmpt.

And a password.

Click the "Confirm Password" field.

The patient will select their preferred day of the week to complete questions.

Click "SIGN UP"

The patient will receive a "Success" message. Click "CLOSE"

The baseline set of ePRO questions will then be presented to the patient for their responses to be recorded
