Using the Generator

You will be taken to the Survey Generator page

Use the dropdown menu on the right to select 'Yes' for the Programming Types that apply to the group completing the survey, and 'No' for those that don't

The Form Link, QR Code and PDF Pages will adjust depending on which Programming Types you select
Accessing the Online Form

Once you’ve selected the Programming Types you need, you can copy the generated Form Link to access the online form.

Click the form link and press Ctrl+C, or right-click the form link and select 'Copy'

Paste the form link into your browser

The online survey will open with the selected Program Types from the generator already filled in

You can also use the generated QR code to easily provide group participants with access to the online form
Printing the Form

To print the survey, open the universal survey PDF by clicking the PDF link shown. You can hover your mouse over the link if needed to reveal the clickable area

Once the PDF opens, you will need to click the three dots on the top left corner

Then click 'Print'

After you are redirected to this page, click the print icon in the top right corner

Before printing, make sure to use the page numbers from the generator to limit the printout to only the pages you need. Click the 'Pages' dropdown to enter the custom page numbers.

Select 'Custom'

This will provide you with a box to enter specific page numbers. Copy the generated page numbers from the form generator and paste them into this box.

You’ll find the page numbers listed under 'PDF Pages' next to the PDF link. Click the box and copy the page numbers

Paste the copied page numbers into the custom pages box

Then you're good to go ahead and print!

If you select a combination of Programming Types that doesn’t yet have a form version available, you’ll see the message shown below. If you need a new combination of Programming Types added, please submit a help desk ticket — we’ll get it added for you!
