Grant - How to associate a Part 2 assessment to a Concern file

To submit an assessment, first go to your Outreach dashboard (<https://outreach.ctipractices.com/>). Click the "Visit Form Filler" button.

On the Form Filler page, you are able to continue from a preexisting assessment. These are listed as "Drafts" under your "Recent Form Drafts" list.

To begin a new Part 2 assessment, click the "Start New Forms and Assessments" button.

Select the Part 2 assessment to begin.

Complete the assessment as needed, filling out the required field and using the "Previous" and "Next" buttons on the bottom navigation bar to move between pages (if there are multiple pages). Note that the "Submit" button will not be able to be selected until all required fields on the assessment are completed.

When all required fields have been entered as necessary and you are considering the assessment complete to be submitted, click the "Submit" button.

Since this an additional assessment for a preexisting file, select or search for the preexisting Concern file.

Click "SUBMIT" after you have selected the relevant Concern file.

On this page, you can either download the submitted assessment or visit the "Concerns" screen.

On the "Concerns" screen, we see all Concern files that you have access to at your school. Select the title of any Concern to open.

You can view details of the file here. Note that both assessments will be listed on this screen and are now linked to this Concern file. Examples of what you are able to do on this screen: Upload and download any files, view and download any assessments, and view any associated individuals/students.
